All  health forms (physicals & updates) are to be uploaded through the portal.  (Link Below )  Athletes are required to submit/upload a copy of their current physical each season, regardless if they had a current physical on file during previous season.  

Please do not hand to a coach or email the registrar.  

All athlete medical forms must be completed and reviewed by the registrar every season before the athlete may participate in any Row New Jersey competitive racing or our winter training program.  Learn-to-Row participants are not required to submit a physical.

NEW AND RETURNING ATHLETES MUST PROVIDE AN ELECTRONIC COPY OF THEIR PHYSICAL COMPLETED ON THE REQUIRED NJ PREPARTICIPATION ATHLETIC PHYSICAL FORM  (available for download on this page-) EACH SEASON

Please note that physicals from clinics (such as CVS) that are not completed on the NJ Preparticipation Athletic Physical Form will not be accepted. 

If the athlete is returning and has had a physical in the last 12 months but HAS HAD ANY MATERIAL CHANGES IN THEIR HEALTH OR HEALTH CARE, a ROW NJ Health Update Form must be signed by the athlete's parent and physician.(available for download on this page. 

All scanned medical forms  should be uploaded through the season secure upload portal found on our website.  Questions related to health forms should be directed to our Registrar- Registrar@RowNJ.org.   DO NOT give the forms to the coaches. DO NOT POSTAL mail the forms.  We encourage you to save the scanned copies of your medical documentation to a folder on your personal device to resubmit for future seasons. 

All adult masters rowers are required to complete, sign and submit the Row New Jersey Risk Waiver and the Masters Rowing Waiver found above.

Use the expense form  to submit approved team expenses for reimbursement.  Receipts are required for reimbursement.  Approval from Activity Chair and Club President required before Treasurer will send payment. 

Electronic submission - email expense form to Treasurer@RowNJ.org and CC the activity chair for approval.  Scanned in receipts are acceptable.  Or you can mail to PO Box 263 Mount Arlington, NJ 07856 attention: Treasurer (please note mailbox is monitored once weekly)
