•  Download the “Physical Form” and the “Medical Eligibility Form”
• Contact your healthcare provider to schedule a physical and  have them complete the required “Medical Eligibility Form” prior to the start of practice.
• Upload the completed “Medical Eligibility Form” via the Row New Jersey Portal (be sure to select the proper portal depending on your program Juniors (8th-12th competitive team) or Middle School Program (6th-8th graders)

UPON registration, our registrar will confirm that you have a valid physical on file.  If you do and it is valid for the season period December 9, 2024 – Feb 28, 2025 your next step is to complete and upload the “Health History Questionnaire”.  The questionnaire is located on our website along with the upload portal.

IF your physical expires within the season period  please follow the next steps:

• Complete and Upload the Health History Questionnaire
• Schedule your athlete’s next physical as soon as possible.  Email registrar@RowNJ.org to notify date of scheduled physical.
• Download the NEW “Physical Form” and the “Medical Eligibility Form” from our website and take these with you to the actual physical.  The Healthcare provider will keep the “Physical Form” and you will ask for the completed “Medical Eligibility Form”. 
• Upload the completed “Medical Eligibility Form”   via the RNJ portal.

ALL NEW PHYSICALS should be completed on the updated PHYSICAL FORM found on our website.  YOU WILL NOT UPLOAD A PHYSICAL TO ROW NEW JERSEY.  Row New Jersey is required to receive a completed “MEDICAL ELIGIBILITY FORM” annually and a completed  “Health History Questionnaire” each season.  We will not accept paper copies of the documentation.  DO NOT HAND any medical forms to our coaches.  Questions regarding the process should be directed to Registrar@RowNJ.org.

We are aware that athlete physical appointments are not always able to be scheduled within 365 days to the day due to insurance policies and health care provider schedules.  Please remain in contact with  our registrar via email.  Row New Jersey has the health & safety of our athletes in mind following the the guidelines for athlete safety.  We appreciate your diligence in keeping your athlete up to date.
